Susan Williams

Sue received her diploma in nursing from Lutheran Hospital School of Nursing in1972.

She worked as a medical-surgical staff nurse/charge nurse at St. Francis Hospital from 1979 to 1987. She then worked as a case manager for St. Francis Home Health, then as an OB/GYN staff nurse/ charge nurse.

She became a staff nurse in the Diabetes Center at St. Francis Hospital in1989, and was certified as a Diabetes Educator in 1992. She obtained ADA Recognition for St. Francis Hospital’s Diabetes Education Program in1997 and has maintained that certification for 15 years. She has been a Certified Nurse Diabetes Educator for 20 years. She is responsible for providing the education and support that people with diabetes need in order to manage their condition and stay healthy. She has been a speaker at several community seminars and has coordinated many seminars for people with diabetes with over 300 people attending these seminars.

Theresa Boswell recently graduated from the University of Wisconsin- Milwaukee with a Ph. D. in Counseling Psychology.  She is currently completing her postdoctoral training to become a licensed Psychologist. 

Dr. Boswell provides psychotherapy in a large metropolitan hospital to children, adolescents and adults.  Dr. Boswell practices as a generalist, which means she is able to treat a variety of mental health issues.  She provides services through Cognitive Behavioral techniques and brief Solution- Focused approaches. 

Dr. Boswell has a master’s in Social Work from the University of Wisconsin- Madison and is a Licensed Clinical Social Worker.  Dr. Boswell has extensive experience dealing with trauma, Anxiety and Depression resulting from health related issues, parental and family conflict, and school adjustment and transitional issues with children and adolescents. 

Dr. Boswell has a unique background with a diversified set of experiences.  She is a previous founder and Executive Director of a small outpatient mental health clinic; she also has experience working on a Crisis Stabilization team, and currently teaches General and Abnormal Psychology courses as an adjunct instructor for adult learners through a technical college system.

 Dr. Malmsten sees patients for a diagnosis and treatment of cardiovascular illnesses and diseases. She believes in using evidence based medicine to provide the best cardiovascular care to her patients and in treating patients the way she would want her family to be treated, with dignity and respect.

Dr. Rosenthal is the interim chief of rheumatology. She sees a variety of rheumatology patients.

Mary Sinnen received her Doctorate in Nursing Practice from St. Catherine University in 2010.  Since 2003, she has been the Director of Nephrology Services at Wheaton Franciscan Healthcare as well as a part-time Nurse Practitioner for Milwaukee Nephrologists, SC.

Mary assisted in the development of a successful critical pathway driven case management model at St. Michael Hospital.   She further extended that model to a “beyond the walls” model in which professional nurses intervened with medically complex patients to keep them from multiple acute care admissions. This opportunity also included nation wide presentations and consultations to hospitals on the application of the model into their unique environments.

During her Director of Nephrology position, multiple accomplishments have been realized, including anemia management clinics; system wide attention to evidenced based practice for chronic kidney disease; and attention to behaviorally challenged dialysis patients.

Graciela Fendt, RN, BSN

Ms. Fendt graduated from Marquette University College of Nursing with a Bachelor in the Science of Nursing in 1997.  After her graduation, she has worked first as a staff nurse, later as a pediatric nurse case manager and since 2000 she has worked as an HIV case manager here at Sixteenth Street Community Health Centers (SSCHC). 

She has performed the role of Bi-lingual Spanish/English HIV Nurse Case Manager for the past twelve years at (SSCHC). In that role she assessed the medical and mental needs of this population and has provided education about HIV disease and progression, medication adherence etc., for adult patients and their significant other and their families.

She is very passionate and committed to working with the HIV population.  She always finds joy in extending her hand to people in need.

Hugo Jimenez, HIV Prevention Coordinator
Mr. Jimenez has worked at Sixteenth Street Community Health Centers for over 10 years and has 5 years of experience working in the field of HIV. He provides education about HIV prevention, HIV counseling and testing in the community in the south side of
Milwaukee, focusing in the LGBT Spanish speaking communities and other communities at higher risk for HIV infection. He is bilingual and is certified by the State of WI DHS HIV/AIDS Program as HIV Counselor and Tester.

Physician Assistant Nicole Evert joined Great Lakes Dermatology in April. Nicole earned her Bachelors of Science degree from the University of Minnesota – Twin Cities. She then went on to complete a Master’s degree in Physician Assistant Studies from Alderson-Broaddus College in West Virginia in 2012. Nicole’s primary goal is to ensure her patients have the healthiest skin possible. She achieves this by attentively listening to the needs of her patients, and providing appropriate treatment options to safely and effectively achieve optimal results. She enjoys treating patients of all ages and skin types. Nicole sees patients in our Racine, Bay View, and Milwaukee clinics.

Jill Fhields is a Certified Laser Technician and a Certified Laser Safety Officer. Her training was done at the National Association of Laser & Aesthetics in Scottsdale, Arizona. When she is not at work, she enjoy spending time outdoors with her  husband and six children. Jill takes great pride in providing her clients with the professional and personal attention they deserve while helping them feel beautiful!

Kelly Richards, MSN, RN, CNS, FNP-BC is a Nurse Practitioner for a local community based primary care clinic and a community based breast health outreach and education program. Ms. Richards graduated from the University of Wisconsin-Milwaukee in 2008 with a Master of Science degree as a clinical nurse specialist with a focus in women’s health education to take control of their health with the ultimate goal of reducing the incidence and mortality of disease among underserved populations.

Kathleen Henley, RN.  Kathleen graduated from the Los Angeles County Medical Center School of Nursing in 1984 and has been an Emergency nurse since that time.  She began her career locally at St. Michael Hospital in Milwaukee after relocating from the Los Angeles area in 1995. She is currently Director of Nursing at Wheaton Franciscan-Elmbrook Memorial and Wheaton Franciscan-Wauwatosa for the Emergency and Urgent Care Departments.  Wheaton Franciscan-Elmbrook Memorial is a Primary Stroke Center and she is an active member of the multidisciplinary team that supports that certification.

Shaneé has been a human service professional for more than 18 years working for organizations that serve and benefit youth and families.  She is also a certified in anger management counseling for youth. She has been with the Y since 2006 serving in various capacities to include Senior Program Director, Director of Operations for the YMCA Holton Youth Center, Associate Executive Director of the Northside YMCA and in her current role as Executive Director of the Urban Campus which includes the oversight of Northside, Parklawn, Centers and Downtown YMCA Urban Programs. She is responsible for the day to day operations of a high quality, member centered YMCA’s and serves as the principal liaison and relationship builder with the local community which includes financial development and member satisfaction.  Additionally, Shanee’ provides leadership to the Sponsor A Scholar mentoring program, Black Achievers , Community Development Center and the Youth Leadership Academy.

Shaneé demonstrates the YMCA’s creed of strengthening the foundations of community outside of her professional realm as well. She believes in exhibiting a true philanthropic philosophy and believes in service to all. Additionally, she continues to expand community relationships as a committee member and volunteer with several organizations to include Whitefish Bay Dominican High school, Golda Meir PTO member, Alpha Kappa Alpha Sorority Incorporated executive committee member, and Jack and Jill of America Incorporated Milwaukee Chapter Vice President. She also serves on the Clear Channel Communication Board and NAACP youth advisory board. Shaneé is active in her church Parklawn Assembly of God where she currently serves in the ministry of helps.

When she is not working or involved in service within the community, both Shanee’ and her husband Derick spend time traveling to various sporting activities for their daughters- Deysha(19)- Freshman at the University of Connecticut(UCONN) and Sydnee(13) 8th grade student at Golda Meir Gifted and Talented School.

Shanee’ is also an entrepreneur and has a registered event planning company as well.

Shaneé earned her Bachelor of Science degree in Criminal Justice from the University of Wisconsin- Milwaukee in 1998.
